Kingdom Hearts 'Heart' flowchart (SPOILERS)

I have too much time on my hands sometimes. I have gone though and created a flow chart showing whenever someones heart changed bodies, someone was split in two, etc. Follow along with the numbers to see the explanations. I doubt this is perfect, and if you see a correction, please let me know WITH PROOF. I don't wish to go on a wild goose chase.

1. BBS - Ventus is split in two halves, light/dark, by his master Xehanort. The evil part becomes known as Vanitas
2. BBS - After the split, Ventus was nearly dead, a young Sora gives part of his heart to bring Ventus back to life.
3. 3D - Xehanort puts part of his heart into Vanitas, becoming one of the thirteen Xehanorts
4. BBS - Ventus defeats Vanitas and they recombine, doing so nearly kills Vanitas
5. BBS - Vanitas’ heart leaves its body and finds refuge in the young Sora’s heart
6. KH1 - The door on Destiny Island opens and Kairi is threatened so her heart hides itself inside Sora
7. KH1 - Sora opens his heart with keyblade of darkness and releases Kairi’s heart so it may return to her own body.
8. KH2 - Upon releasing Kairi’s heart, he accidentally created Namine, a strange nobody who is both Kairi’s nobody and Sora’s.
9. KH1 - By releasing Kairi’s heart, he also released his own, where it became a heartless
10. KH2/BBS - Upon becoming a heartless, Sora’s will was strong enough that he became a nobody known a Roxas. It is also believed that he was partially Ventus’ heart, which explains his appearance.
11. KH1 - Kairi recognizes Sora’s Heartless as Sora, and gives him back part of herself to bring him back to human.
12. KH2 - Namine joins back together with Kairi in The Castle That Never Was, allowing her to be whole again
13. KH1FM/358/2 - In the Final Mix version of KH1, there is an extra battle with Xemnas in Hollow Bastion, after which, Xemnas uses a sample of Sora to create a puppet based upon him, Xion.
14. 358/2 - Xion slowly sucked the life out of Roxas as they worked in Organization XIII together. In a final clash of will, Roxas destroys his best friend and sadly absorbs her partial heart back into his.
15. KH2 - Roxas returns to Sora in order to awaken him from his sleep, completing his heart.
16. BBS - After Terra defeats his old master, Eraqus, Xehanort kills him. Eraqus’ heart then finds its way into Terra to help protect him against the darkness within him.
17. BBS - Xehanort corrupts Terra until he is full of darkness, then releases his own heart and enters Terras body. After a long internal fight, all three hearts are put into stasis as the body lives with only skattered memories from Xehanort and Terra’s lives.
18. BBS - Within the internal struggle, the dark keyblade is used again to try and release a heart from the body. Whose heart is expelled here is currently a mystery.
19. KH2 - Xehanort uses his dark keyblade to split himself into Nobody and Heartless. The nobody became Xemnas, the leader of Organization XIII.
20. KH2 - Xehanort uses his dark keyblade to split himself into Nobody and Heartless. The heartless became Ansem, the villian of the KH1.
21. 3D - Braig, after helping Xehanort in his plans is turned into a nobody by Terranort. His name was changed to Xigbar.
22. 3D - Isa, was tricked into being turned into nobody by either Braig or Terranort (presumably). His name was changed to Siax.
23. 3D - Xemnas put part of his soul into Xigbar, creating one of the thirteen Xehanorts.
24. 3D - Xemnas puts part of his heart into Siax, creating one of the thirteen Xehanorts.
25. KH1 - Riku, wishing for the power to defeat Sora and protect Kairi, accepts the weakened Ansem into himself, become imbued with the powers of darkness.
26. KH1 - After Riku is defeated, Ansem emerges from him, fully powered by his rest inside his heart.
27. CoM - Vexen samples Riku’s heart and creates a replica to fight Sora and the heartless.
28. KH2 - After DIZ’s machine explodes, Riku is able to shed Ansem’s hold on his heart.
29. 3D - When attacked on their way to the dreaming worlds, Riku hides himself inside Sora’s heart and instead runs around as a Dream Eater.
30. 3D - After defeating Ansem in the sleeping worlds, Riku awakens from his sleep and returns to his own body.
31. 3D - Riku imediatly turns around and jumps back into the sleeping worlds to save Sora from the thirteen Xehanort’s and defeat Young Xehanort.
32. 3D - After defeting Young Xehanort, Riku returns to his body once again to find Sora safe and sound.
